Objectives Access to healthcare in most western societies is based on equality. Rapidly rising costs have fuelled debates about differentiation in access to healthcare. We assessed the public's perceptions and attitudes about differentiation in healthcare according to lifestyle behaviour. Methods A vignette study was undertaken in participants in a colorectal cancer screening pilot programme in the Netherlands. Screenees with a negative test result received a questionnaire in which nine hypothetical situations were described: three different healthcare settings (screening, lung cancer, chronic (...) obstructive pulmonary disease) combined with three forms of differentiation each: a difference in premium, waiting list ordering or copayment according to lifestyle. We evaluated the responses using a general hierarchical linear model. Results The percentage of participants in agreement with differentiation varied from 20% to 58% (overall mean of 40%). Significantly more participants were in favour of giving a premium discount to those who do not engage in unhealthy behaviour compared with supporters for higher payments for those who do. More differentiation was supported for non-smoking versus smoking cessation than for participation versus non-participation in screening. We observed in-group favouritism in smokers, but no significant effects of age or disease. There was no support for waiting list ordering based on lifestyle. Conclusions Results of this study show that Dutch citizens eligible for colorectal cancer screening are in favour of some form of financial differentiation in the distribution of healthcare, but that significant differences exist between type of setting and the type of behaviour. Our study can be used in the ongoing discussion about solidarity and behaviour in healthcare. (shrink)

Sellars claims completeness for both the “manifest” and the “scientific images” in a way that tempts one to assume that they are independent of each other, while, in fact, they must share at least one common element: the language of individual and community intentions. I argue that this significantly muddies the waters concerning his claim of ontological primacy for the scientific image, though not in favor of the ontological primacy of the manifest image. Sellars was committed to the irreducibility of the semantic, the intentional, and the normative. Nevertheless, he was also committed to naturalism, which is prima facie at odds with his other theses. This paper argues that Sellars maintained his naturalism by being linguistically pluralistic but ontologically monistic . There are irreducibly distinct forms of discourse, because there is an array of distinguishable functions that language and thought perform, but we are not ontologically committed to the array of apparently non-natural entities or (...) relations mentioned in the metalanguage. However, there is an underlying relation between language and world presupposed by all empirically meaningful language. In his early work Sellars sought to describe this relation in linguistic terms as a form of 'pure description', but inadequacies in that notion drove him towards the naturalistic relation between language and world that he came to call 'picturing'. Research on expert‐novice differences has mainly focused on how experts solve familiar problems. We know far less about the skills and knowledge used by experts when they are confronted with novel problems within their area of expertise. This article discusses a study in which verbal protocols were taken from subjects of various expertise designing an experiment in an area with which they were unfamiliar. The results showed that even when domain knowledge is lacking, experts solve a novel problem within their (...) area of expertise by dividing the problem into a number of subproblems that are solved in a specified order. The lack of domain knowledge is compensated for by using abstract knowledge structures and domain‐specific heuristic strategies. However, the quality of their solutions is considerably lower than the quality attained by experts who were familiar with the type of problem to be solved. The results suggest that when experts are confronted with novel problems as compared with familiar problems, their form of reasoning remains intact, but the content of their reasoning suffers due to lack of domain knowledge. Rumination is a process of uncontrolled, narrowly focused negative thinking that is often self-referential, and that is a hallmark of depression. Despite its importance, little is known about its cognitive mechanisms. Rumination can be thought of as a specific, constrained form of mind-wandering. Here, we introduce a cognitive model of rumination that we developed on the basis of our existing model of mind-wandering. The rumination model implements the hypothesis that rumination is caused by maladaptive habits of thought. These habits of (...) thought are modeled by adjusting the number of memory chunks and their associative structure, which changes the sequence of memories that are retrieved during mind-wandering, such that during rumination the same set of negative memories is retrieved repeatedly. The implementation of habits of thought was guided by empirical data from an experience sampling study in healthy and depressed participants. On the basis of this empirically derived memory structure, our model naturally predicts the declines in cognitive task performance that are typically observed in depressed patients. This study demonstrates how we can use cognitive models to better understand the cognitive mechanisms underlying rumination and depression. (shrink)

ABSTRACTThe question concerning the relation between thinking and the university is the starting point of this paper. After a very brief outline of some reflections on this topic, the case of Campus in Camps, a Palestinian experimental university, is presented to shed light on this issue. Inspired by Isabelle Stengers’ ecology of practices, it is possible to discern four requirements on thinking in the work of Campus in Camps, namely storytelling, comparing, mapping, and using. It will be argued that the (...) particularity of thinking at the university, is that it is done via artifices that initiate processes of composition, problematization, and attention. In the concluding section, the paper proposes to understand the study practice of Campus in Camps as an adventure that activates a sense of the possible, and hence opens up futures that are different from the ones that present themselves as obvious or unavoidable. An analysis of Hegel's chapter on teleology in the Science of Logic. Hegel argues that the 'intentional model' of teleology assumed by Kant actually presupposes a natural or organic teleology more like along Aristotelian lines.

In many logics dealing with information one needs to make statements not only about cognitive states, but also about transitions between them. In this paper we analyze a dynamic modal logic that has been designed with this purpose in mind. On top of an abstract information ordering on states it has instructions to move forward or backward along this ordering, to states where a certain assertion holds or fails, while it also allows combinations of such instructions by means of operations (...) from relation algebra. In addition, the logic has devices for expressing whether in a given state a certain instruction can be carried out, and whether that state can be arrived at by carrying out a certain instruction.This paper deals mainly with technical aspects of our dynamic modal logic. It gives an exact description of the expressive power of this language; it also contains results on decidability for the language with arbitrary structures and for the special case with a restricted class of admissible structures. In addition, a complete axiomatization is given. The paper concludes with a remark about the modal algebras appropriate for our dynamic modal logic, and some questions for further work. (shrink)

We develop an approach to automatically identify the most probable multiword constructions used in children’s utterances, given syntactically annotated utterances from the Brown corpus of CHILDES. The found constructions cover many interesting linguistic phenomena from the language acquisition literature and show a progression from very concrete toward abstract constructions. We show quantitatively that for all children of the Brown corpus grammatical abstraction, defined as the relative number of variable slots in the productive units of their grammar, increases globally with age.

Where the possibility of metaphysics as a science is concerned, Kant assigns the exact sciences the function of an exemplar; for these disciplines have long been well established on "the secure path of a science." Accordingly, in the Prolegomena Kant explicitly addresses the question "How is metaphysics possible as a science?" by way of the questions "How is pure mathematics possible?" and "How is pure natural science possible?" Moreover, all these questions arise directly out of the main transcendental question: "How (...) are synthetic judgments a priori possible?" The deplorable situation of the metaphysics of the day would have been entirely attributable to misappraisal of this central transcendental question. (shrink)

This third part continues the exploration of how we can overcome the limitations of the present knowledge system. In preparation, two aspects of current engineering theory and practice are examined because they are paradigmatic: the concept that engineering is essentially problem-solving, which goes against our understanding of human skill acquisition, and the existence of parallel modes of knowing technology derived from professional education and practice and from living in a society permeated by technology. The former suspends practitioners in the previously (...) examined triple abstraction as a primary microlevel characteristic of the current knowledge system and is thus separated from experience and culture. The second mode of knowing derives from daily life experience, symbolically represented by the organization of the brain-mind, which also functions as a mental map for skillfully coping with the world according to a culture. The practitioner primarily derives analytical exemplars from the former knowing and design exemplars from the latter. It is shown how this affects the building of cities and the design of production systems. Umbrella concepts and round tables are introduced as further steps toward transforming university departments, and (as we will see in Part 4) the operation of the university. (shrink)

The German end-of-life vehicle take-back system is described and analyzed in terms of its impact on the environment and the car companies involved. It is concluded that although this system is often cited as an example of a successful take-back scheme, it is not one that maximizes the value recovered from end-of-life vehicles. As a result, corporations do not achieve the potential benefits that can be realized from an alternate value chain based on recovering value from end-of-life products. Neither is (...) society benefiting from the potential reduction in pollution and from making its way of life more sustainable. Without bringing these potential benefits to all stakeholders, the system is not likely to have a future. (shrink)

Judgments of the rationality of beliefs must take the costs of acquiring and possessing beliefs into consideration. In that case, certain false beliefs, especially those that are associated with the benefits of a cohesive community, can be seen to be useful for an agent and perhaps instrumentally rational to hold. A distinction should be made between excusable misbeliefs, which a rational agent should tolerate, and misbeliefs that are defensible in their own right because they confer benefits on the agent. Likely (...) candidates for such misbeliefs are to be found in the realm of nationalism and religion, where the possession costs of true beliefs are high, and where collective beliefs in falsehoods may allow for a cohesive community. We discuss the paradoxes of reflective awareness involved in the idea of deliberately embracing falsehoods. More rigorous, fully reflective concepts of rationality would still disallow false beliefs, but such demanding versions of rationality would commit agents to pay large costs, thereby weakening the motivation for acquiring true beliefs. The is a reading of Hegel's chapter on teleology in the Science of Logic. It argues that inadequacies in the intentional model of teleology that dominated both pre-Kantian and Kantian thought about teleology force us to recognize a much more Aristotelian conception of natural teleology that must be presupposed to make sense of the teleology of intentions.

High levels of specialization have created knowledge with little or no “peripheral vision,” and the resulting “blind spots” are causing many “collisions” with human life, society, and the biosphere. Each discipline and specialty must be equipped with a “map” showing its connections to everything else, but especially the negative consequences that tend to be associated with its practices. Preventively oriented practices can improve the ratio of desired to undesired effects of design and decision making to create ways of life that (...) are more sustainable across the board, as opposed to treating symptoms (such as global warming). Certain enterprises at the fringes of science, such as intelligent design creationism, claim to identify phenomena that go beyond not just our present physics but any possible physical explanation. Asking what it would take for such a claim to succeed, we introduce a version of physicalism that formulates the proposition that all available data sets are best explained by combinations of “chance and necessity”—algorithmic rules and randomness. Physicalism would then be violated by the existence of oracles that produce certain kinds (...) of noncomputable functions. Examining how a candidate for such an oracle would be evaluated leads to questions that do not admit an easy resolution. Since we lack any plausible candidate for any such oracle, however, chance-and-necessity physicalism appears very likely to be correct. (shrink)

In the decades to come, the authors expect growing pressures to reform current production systems to make them more compatible with the biosphere. A proactive approach to this pressure involves consideration of an alternate value chain based on a comprehensive engineering and marketing approach to recover value from end-of-life products. To estimate the potential advantages of the new value chain, the authors calculate the minimum throughput advantages and environmental advantages that can be realized from a comprehensive strategy of recovering value (...) from end-of-life products. The efforts of corporations and other organizations to make modern ways of life more environmentally sustainable are evaluated against this benchmark in terms of activities in the area of selling services, product take back, life cycle assessment, Responsible Care, voluntary emission reduction initiatives, and engineering and management education. War of Ideas: On Feyerabend’s Epistemic Pluralism and the Polarising Effect of Alternative Truths Paul Feyerabend is generally known as one of science’s greatest advocates of (epistemic) pluralism. Throughout his work, Feyerabend rejects the notion of a monistic epistemology and defends various forms of pluralism. Consequently, he is sometimes reproached for being responsible for the emergence of alternative truths – a radical form of pluralism – in the current post-truth era. This paper discusses the negative effects of alternative truths and (...) the extent to which a solution for these effects is provided by Feyerabend’s theory of science. After narrowly defining alternative truths, it is concluded that alternative truths can have a polarising effect that may impede the progress of knowledge. At first glance, Feyerabend’s pluralistic theory does not acknowledge this downside of alternative truths. However, this paper argues that Feyerabend’s exclusion of cranks, as well as his (critical) view on reason, can be seen as possible limitations of his (epistemic) pluralism. The problem of alternative truths, therefore, is not necessarily an argument against Feyerabend’s theory of science. (shrink)

The German packaging ordinance is an example of legislated extended producer responsibility (also known as product take-back). Consumers can leave packaging with retailers, and packagers are required to pay for their recycling and disposal. It can be considered to be successful in reducing waste, spurring the redesign of packaging to be more environmentally sustainable, and increasing refilling and recycling. The exception is waste packaging made of plastics, which faces the problems of export due to lack of markets for recycled products (...) within Germany, the lack of capacity for recycling, the allowance for energy recovery within the ordinance, and the manufacturers’ focus on back-to-feedstock rather than back-to-plastics as a solution. This article looks at the ordinance and its effects to consider why this take-back program worked better than others. (shrink)

Contemporary civilization has created a fundamental contradiction between the intellectual approach to knowing and the technical approach to doing on the one hand, and the results of their application on the other. These approaches to knowing and doing begin with a process of abstraction that trades breadth for depth, while their application reveals a multitude of problems associated with ignoring the fact that everything is related to everything else in our world. Scientific disciplines and technical specialties create “pure” domains of (...) investigation “uncontaminated” by interactions with all other domains, thereby distorting the internal fabric of relations and its connections with everything else. Hence, the most powerful means humanity has ever created are indissociably linked with the production of chaos in human life, society, and the biosphere. A new domain of study is needed to the interactions between society, technology, science, and the biosphere (STSB). The scope, direction, and methodological challenges are briefly examined, and the possible mission of STSB within the contemporary university. (shrink)
